Annual Medicaid Redetermination
If you are a member of an AmeriHealth Caritas Medicaid plan, you will be receiving your annual Medicaid eligibility review form. Please complete and return this form as soon as you receive it. If you do not submit it on time, you and your family could lose your health care benefits.
